Please Help: Change of name

I want to change my name in the records of ICAI. Can somebody help me with the procedure and documents required and also the person whom to be contacted. |

| File an affidavit before a public notary for the name change. Publish the same in 2 newspapers. File an application with ICAI for change of name with the original copy of the newspaper containing that publication. ICAI will issue a certificate in letter form certifying the change in name, from that day onwards ur name will stand changed in the records of the institute , but no fresh certificates will be issued for the same.
